Per member per month model to connect patients from their home to their professional care providers. Fee from hardware sales, fee for services including customer support, logistic handling, use of our solution alignment software that aligns digital health devices and solutions to patient populations, and engagement analytic reports.

The Life365 team previously sold their remote patient monitoring company, MedApps, for 30x revenue to a 3bn rapid diagnostic company in 2012. They know the industry well are subject matter experts in mHealth & remote patient monitoring.
